2. Where – Location of missing.
4. How it was missing.
Note : Kindly proceed to PIDS Centre located at One Stop Centre for replacement of staff card. You are also required to contact Security Control Room at 05 3688006 for card inactivation ( to avoid any misuses)
Replacement of staff card is chargeable RM30.00.
