HOW TO APPLY FOR THE STAFF CARD
Please submit your request through the Unified Customer Service (UCS) Portal. You may refer to the guideline provided as below:
- Title: Staff Card
- Type: Request
- Subject: SSD – Security Pass Application
- Your Enquiry: (Please provide the following information)
- Full Name (as per IC/Passport)
- Preferred Name to be displayed on the card (cannot be more than 10 characters)
- Appointment/Job position
- Staff ID Number
- Home Address
- IC/Passport Number
- Contact Number
- Year of appointment
- Start date and month of appointment.
- Last date and month of appointment
- Attach file:
Offer Letter/Appointment letter
- Please upload your passport-sized photograph
- Photo Length - 4.5 cm & width – 3.5 cm
- With either blue or white background
- The subject must be facing forward, looking straight at the camera as per the photo below:
- The initial Security pass is free; however, the subsequent cards will be charged as follow:
- Replacement of damaged card is RM30.00
- RM30 penalty will be imposed on the lost card.
- The payment (RM30:00) can be made online as below:
- Universiti Teknologi Petronas, Bank Account No: 8004857910 – CIMB.
- For reference: Please mention the Staff ID and the title being Payment of Security Pass.
- Please attach the transaction/bank-in-slip upon settlement of Security Pass through the UCS Portal.
- Please proceed to the Security Counter (One Stop Centre), Chancellor Complex for the Security pass arrangement. Counter operation hours are as per below.

- The process to renew the Security pass will be the same as a new application.
